Gather your medical information now

Our Emergency Information forms will help you record your most important medical information before you need an ambulance.

Print out and complete a form for each member of your family. Put the forms in a place that's easy for us to find, such as the refrigerator.

Someone is sick or hurt
... should you call 9-1-1?

If you're not sure whether a situation is serious enough to need an ambulance trip to the hospital, the answer is simple:

When in doubt, call 9-1-1!

If you do not need to go to the hospital, there is no charge for an ambulance visit. If we and the hospital feel it's safe for your family or friends to drive you there, then you may do that. But if you do need immediate hospital care, an ambulance trip just might save your life.

How to help us help you

When we arrive, we'll need to know the patient's important medical information:

  • Anything you can tell us about this problem
  • The patient's significant medical conditions
  • The name of the patient's doctor - especially a cardiologist (heart doctor)
  • Medications that the patient is taking, even over-the-counter drugs or supplements. If you don't have these written down, gather the bottles for us to see.
  • Does the patient have allergies to any drugs?
  • When did the patient last eat or drink?
